Behandling af dine personoplysninger – samtykke

Jeg giver hermed mit samtykke til at klinisk psykolog Liselotte Mosbæk må indsamle og behandle mine personoplysninger. Personoplysninger omfatter følgende typer:

• Navn
• E-mail adresse
• Telefonnummer
• CPR-nummer
• Helbredsoplysninger til brug i vores arbejde
• Notater fra vores sessioner

Jeg er endvidere indforstået med, at sessioner kan støttes af en AI-assistent, der optager lyd, laver et transskript i realtid (notetagning) og genererer et udkast til journalnotat. Terapeuten er fortsat ansvarlig for den endelige journalføring, og lydoptagelser bliver ikke gemt.

Oplysningerne opbevares elektronisk hos Terapeut Booking (www.terapeutbooking.dk).

Processing of your personal information - Consent.

I hereby consent to the processing of my personal data by test. Personal information includes the following types:

• Name
• E-mail address
• Contact information
• National Insurance number
• Health information
• Notes from our sessions

I further agree to the sessions being supported by an AI assistant that records audio, creates a real-time transcript (note taking), and generates a draft journal note. The therapist remains responsible for the final journal entry and audio recordings will not be stored.

All information is stored electronically at EasyPractice (www.easypractice.net).
