Consent Form to Collect & Use your Personal Information

Under the General Data Protection Regulation (GDPR) there are occasions the Health Optimizing Clinic in Bergen must obtain your explicit consent to process your information, known as ‘Personal Data’. Personal Data includes, but is not limited to your name, address, telephone number, email address, health history.

As part of our daily process for our clinical services, we need to process personal information about our clients to support an efficient administration process and perform our Health Optimizing services.

If you do not consent to the Health Optimizing Clinic in Bergen using your information for the purposes listed below we’re unable to provide you with our products or services if you do not provide certain information to us. In cases where providing some personal information is optional, we’ll make this clear.

We will use your personal data to the purposes that are listed below, but is not limited to them.

• To perform the health assessments and treatments in the Health Optimizing Clinic;
• To include our contact details in the Health Optimizing Clinic in Bergen clients` databases;
• To keep you informed of news, services, activities and events at the Health Optimizing Clinic in Bergen;
• To contact you to follow up and/or remind you about new assessment, treatments, supplements refill etc.;
• To share your contact details with other Health Optimizing Clinics if necessary so that they can contact you with regards to events or services which are relevant to you.
• To transfer your personal information outside the European Economic Area in a rare occasions to get a consultation from non-EU based specialists. If we do so we'll make sure that suitable safeguards are in place, for example by using approved contractual agreements, unless certain exceptions apply.
• You consent to the Health Optimizing Clinic in Bergen contacting you for the above purposes by: post, email, phone, SMS, social media including Facebook, etc.

All patient data are stored in our systems for 5 years after the year ends or until a patient submits a takedown request of their data by contacting us or our Data Protection Officer.

You can change your consent preferences or withdraw your consent completely by submitting a clear and well-written email to us or our Data Protection Officer (see contact information at bottom of document) outlining your personal information, the extent of the takedown (i.e. what data) and if possible, any reasoning behind requesting the takedown.
