Dette er en opsummering af alle de data vi behandler, både ved tidsbestilling og ved undersøgelser. Hvis du ikke giver samtykke ved bookingtidspunkt, vil du ved ankomst i klinikken blive bedt om at udfylde en skriftlig samtykkeerklæring - dette gælder uanset om du har givet elektronisk samtykke eller ej, da det skriftlige skema indeholder spørgsmål, som er relevante for din undersøgelse og individuelle for dig.

Jeg giver hermed mit samtykke til at Spire - Klinik for graviditetsscanning må indsamle og behandle mine personoplysninger i bookingsystemet Terapeut Booking. Jeg giver endvidere mit samtykke til, at Min Spire ApS kan opkræve mig 50% af prisen for den scanning der er booket, hvis jeg udebliver eller aflyser mindre end 12 timer før min aftale.

Personoplysninger omfatter følgende typer:

Navn, e-mail og telefonnummer. Du kan til enhver tid anmode om at blive slettet fra systemet igen. Dine data vil automatisk blive slettet efter 2 år med inaktivitet. Vi opbevarer ikke sundhedsfaglige oplysninger eller CPR-nummer i bookingsystemet.

Når du ankommer i klinikken indsamler vi nogle oplysninger til brug for vurdering af dit barns vækst og velbefindende, herunder oplysninger om din graviditet, dit navn og dit CPR-nummer. Af denne grund vil du blive bedt om at udfylde en samtykkeerklæring. CPR og helbredsoplysninger er kun tilgængelige for sundhedsfagligt personale tilknyttet klinikken, benyttes udelukkende til journalføring lokalt i klinikken og videregives ikke til tredjepart. Egen læge og fødested orienteres ikke rutinemæssigt om undersøgelsesresultater fra Spire. Vi videregiver kun oplysninger til relevant sundhedsfagligt personale ved indikation og efter klar aftale med dig. Vi indsamler disse oplysninger, da vi som autoriseret sundhedspersonale er forpligtede til at dokumentere og journalføre vores arbejde.

Vi benytter yderligere dit telefonnummer og e-mailadresse mhp kontakt med dig inkl. overførsel af billeder og information fra din scanning. Oplysningerne i din graviditetsjournal opbevares i 5 år jf. gældende lovgivning og scanningsbillederne i op til 3 måneder, hvorefter de slettes. I denne periode kan du til enhver tid anmode om adgang til dine data, og/eller sletning af de data, som vi ikke er lovmæssigt forpligtet til at gemme, fra vores systemer. Sletning af data indebærer, at adgang til journalføring og billedmateriale bortfalder og ikke kan genskabes. Vi er lovmæssigt forpligtede til at opbevare alle transaktionelle data, inkl. kontakt- og firmaoplysninger samt journaler i fem år.

Når du har besøgt klinikken, vil du sandsynligvis modtage en invitation til at bedømme os på Trustpilot. Det er op til dig, om du vil benytte dig af dette og du kan afmelde dig fra Trustpilot via link nederst i invitationsmailen. Vi tilstræber at sende en invitation ud til alle vores besøgende. Hvis du ikke modtager en invitation er du meget velkommen til selv at anmelde os her.

Indsamling af ovenstående oplysninger får vi direkte fra dig eller den som booker din tid.

Min Spire ApS indsamler kun de personoplysninger, der er nødvendige for at levere vores ydelse. Min Spire ApS indsamler og/eller køber ikke personhenførbare oplysninger fra tredjepart.

Min Spire ApS benytter NETS til håndtering af online korttransaktioner og opbevarer ikke informationer om betalingskort. Bemærk at ved fakturering opbevares fakturerings- og bankoplysninger i henhold til gældende love på området herunder hvidvasknings- og bogføringsloven.

Consent to processing personal data

This is a summary of all the data we process both when you book an appointment as well as when we examine you in the clinic. If you do not give consent at the time of booking,  you will be asked to fill in a written consent form upon arrival at the clinic - this applies regardless of whether you have given electronic consent or not, as the written form contains questions that are relevant for your examination and individual for you.

I hereby give my consent for Spire - klinik for graviditetsscanning to collect and process my personal data in the booking system ‘EasyPractice'. Additionally I give my consent that Min Spire ApS can charge me 50% of the price for the scan booked if I do not show up or cancel less than 12 hours before my appointment.

Personal data includes the following types:

Name, e-mail address and phone number. You can request to be deleted from the system at any time. Your data will be automatically deleted after 2 years of inactivity. We do not store healthcare information or CPR number in the booking system.

When you arrive at the clinic we collect some information for use in assessing your baby's growth and well-being, including information about your pregnancy, your name and your CPR number. For this reason you will be asked to complete a written consent form. CPR and health information are only available to healthcare staff associated with the clinic, are used exclusively for record keeping locally in the clinic and are not passed on to third parties. GP and place of birth are not routinely informed about examination results from Spire. We only pass on information to relevant healthcare personnel when indicated and after a clear agreement with you. We collect this information as we, as authorized healthcare personnel, are obliged to document and record our work.

We use your telephone number and e-mail address for contacting you and transferring images and information from your scan. The information in your pregnancy record is stored for 5 years, according to current legislation, and the scan images for up to 3 months, after which they are deleted. During this period, you can request access to your data and/or deletion of the data that we are not legally obliged to store from our systems at any time. Deletion of data implies that access to record keeping and image material ceases and cannot be recreated. We are legally obliged to store all transactional data, including contact and company information referring to these as well as records for five years.

After visiting the clinic, you will probably receive an invitation to rate us on Trustpilot. It is up to you whether you want to do this and you can unsubscribe from Trustpilot via the link at the bottom of the invitation email. We aim to send out an invitation to all our visitors. If you do not receive an invitation, you are very welcome to review us here.

We collect the above information directly from you or the person booking your appointment.

Ethnicity for nuchal translucency scans (necessary for the risk calculation)

CPR number when you arrive at the clinic

For Panorama Test you must consent to us shipping the samples to the United States (outside of Europe)

For Evita Test Complete, you must consent to us shipping your samples to the clinical genetics department at Skejby Hospital

For Doubletest (nuchal translucency scan) your blood sample is analyzed at Rigshospitalet and you can access the result at sundhed.dk if you have a Danish CPR and access to ‘Sundhedsplatformen’

Personal information submitted on https://www.minspire.dk or via telephone contact is only passed on:

Internally at Min Spire ApS

Trusted third parties that are necessary for booking, invoicing, payment and servicing including blood tests, debt collection and legal entities

Min Spire ApS only collects the personal data that is necessary to deliver our service. Min Spire ApS does not collect and/or purchase personally identifiable information from third parties.

My Spire ApS uses NETS for handling online card transactions and does not store information about payment cards. Please note that when a bill is created invoicing and bank information is stored in accordance with applicable laws in the area, including the Money Laundering and Bookkeeping Act.
