Oplysninger i henhold til EU's databeskyttelsesregler

Her er nogle oplysninger, jeg skal give dig i henhold til EU's databeskyttelsesregler. Du bedes læse dem grundigt igennem, før du giver dit samtykke.

Som psykolog og psykoterapeut er jeg forpligtet til at føre journal. Det betyder, at jeg registrerer dit navn, din adresse og dit telefonnummer og, hvis det fremgår af din profil, dit CPR-nummer. Hvis du går i terapi hos mig, skriver jeg journalnotater, der indeholder personlige oplysninger om dig, dine sociale forhold, dit helbred og det, vi har talt om i dine sessioner. Formålet er at kunne give dig en god og relevant behandling.

Jeg kan modtage oplysninger om dig fra din læge og andre, hvis du har givet dit samtykke til, at de sender oplysningerne til mig. Jeg kan videregive oplysninger til din læge og andre, hvis du har givet dit skriftlige samtykke til det.

Jeg har tavshedspligt, og det er kun mig, der har adgang til din journal. Jeg opbevarer din journal i mit bookingsystem, som er en krypteret platform hos TerapeutBooking.dk. Jeg har to-faktor-godkendelse på mit login til systemet. Jeg opbevarer dine oplysninger i systemet i 5 år og i særlige tilfælde længere. Efter denne periode slettes din journal fuldstændigt.

Ved at give samtykke er du også indforstået med, at sessioner kan støttes af en AI-assistent, der optager lyd, laver en transskription i realtid til brug for notetagning og genererer et udkast til journalnotat. Jeg er fortsat ansvarlig for den endelige journalføring. Lydoptagelser gemmes ikke.

Jeg har pligt til at bryde min tavshedspligt, hvis der er alvorlig risiko for, at du eller andre kommer til skade.

Jeg har pligt til at underrette kommunen, hvis jeg er bekymret for et barns trivsel og udvikling.

Jeg sender oplysninger til betalere, hvis nogen – f.eks. sygesikring, forsikring eller kommune – betaler dine samtaler helt eller delvist.

Jeg sender dit navn og oplysninger om, hvornår du har været hos mig, til min bogholder/revisor, så jeg kan få udarbejdet mit årsregnskab. Min bogholder/revisor har tavshedspligt, og vi har en databehandleraftale om korrekt håndtering af data.

Du har ret til at få at vide, hvad der står i din journal. Kontakt mig, hvis du ønsker en kopi. Hvis noget er forkert i din journal, kan vi tilføje en rettelse.

Hvis du er utilfreds med, hvordan jeg håndterer dine oplysninger, bedes du orientere mig. Du har ret til at klage til Datatilsynet over min håndtering af dine personoplysninger.

Du har også ret til at trække dit samtykke til opbevaring af dine data tilbage.

Jeg besvarer ikke mails eller sms'er vedrørende din terapi eller personoplysninger, da disse kommunikationsformer ikke er sikkert krypterede. Hvis jeg modtager sådanne beskeder, sletter jeg dem. Du bedes i stedet skrive til mig via mit bookingsystem, som tilbyder krypteret og fortrolig kommunikation med mig.

Consent and information about how I handle your personal data

Information under EU data protection rules

Here is some information I am required to provide under EU data protection rules. Please read it carefully before giving your consent.

As a psychologist and psychotherapist, I am required to keep clinical records. This means that I register your name, address and telephone number and, if it is included in your profile, your Danish CPR number. If you are in therapy with me, I write clinical notes containing personal information about you, your social circumstances, your health and what we have discussed in our sessions. The purpose is to provide you with appropriate and relevant treatment.

I may receive information about you from your doctor or others if you have consented to them sharing this information with me. I may share information with your doctor or others if you have given your written consent for me to do so.

I am bound by professional confidentiality, and only I have access to your clinical records. I store your records in my booking system, which is an encrypted platform provided by TerapeutBooking.dk. I use two-factor authentication to access the system. I store your information in the system for 5 years and, in special circumstances, for longer. After this period, your records are deleted completely.

By giving your consent, you also understand and agree that sessions may be supported by an AI assistant. The AI assistant records audio, creates a real-time transcription for note-taking purposes, and generates a draft of the clinical record. I remain fully responsible for the final clinical record. Audio recordings are not stored.

I have a duty to break professional confidentiality if there is a serious risk that you or someone else may be harmed.

I have a duty to notify the municipality if I am concerned about a child's well-being and development.

I provide information to third-party payers if someone – for example, a health insurance provider, insurance company or municipality – pays for all or part of your sessions.

I provide your name and information about when you have attended sessions to my accountant/auditor so that my annual accounts can be prepared. My accountant/auditor is also bound by confidentiality, and we have a data processing agreement concerning the proper handling of data.

You have the right to know what is contained in your clinical record. Contact me if you would like a copy. If something in your record is incorrect, we can add a correction.

If you are dissatisfied with how I handle your personal information, please let me know. You have the right to lodge a complaint with the Danish Data Protection Agency (Datatilsynet) about how I process your personal data.

You also have the right to withdraw your consent to the storage of your data.

I do not respond to emails or text messages concerning your therapy or personal data, as these forms of communication are not securely encrypted. If I receive such messages, I delete them. Please contact me through my booking system instead, which provides secure and confidential communication with me.

Processing of your personal data – consent

I hereby consent to Karin von Daler Healing Arts collecting and processing my personal data.

Personal data includes:

• Name
• Email address
• Contact information
• Danish CPR number
• Health information and social and other personal information I have shared with Karin von Daler during my therapy sessions

The information is stored electronically with TerapeutBooking.

Withdrawal of your consent

You can withdraw your consent at any time. You can do so by contacting Karin von Daler Healing Arts by email at: karin@karinvondaler.com.

Additional information about the processing of personal data

LAWS AND REGULATIONS:

Clinical records:
Chapter 6 of the Danish Authorization Act and the Journal Order (Executive Order no. 1090 of 28 July 2016 concerning clinical records kept by authorized healthcare professionals).

Consent to the processing of data:
Article 6(1)(b) and (f) of the EU General Data Protection Regulation (GDPR).

Duty to report serious risk of harm:
Section 41 of the Danish Health Act.

Enhanced duty to notify the authorities concerning the well-being of children and young people:
Section 153 of the Danish Social Services Act.
