• Name
• E-mail address
• Contact information
• National Insurance number
• Health information

All information is stored electronically at Terapeut Booking (https://terapeutbooking.dk/).

Consent to Online Psychotherapy and Psychiatry

By checking this box, I confirm that:

• I consent to receiving psychotherapy and/or psychiatric treatment online (e.g. via video or telephone).
• I understand that online treatment differs from in-person care and may involve technical limitations.
• I acknowledge that the clinic does not provide emergency or crisis services, and that in acute situations I must contact local emergency services, my general practitioner, or a psychiatric emergency unit.
• I understand that information from my treatment will be documented and handled confidentially in accordance with applicable health legislation and the General Data Protection Regulation (GDPR).
• I am aware that I may withdraw my consent at any time.
