HÅNDTERING AF PERSONOPLYSNINGER HOS KONPSYK 

Velkommen hos os. Her er nogle oplysninger, vi skal give dig ifølge EU’s Persondatalov: 

Vi har journalpligt. Det vil sige, at vi noterer og opbevarer oplysninger om dig (f.eks. navn og cpr.nr.) og det, vi taler om (fx dine sociale forhold og dit helbred). Formålet er at give dig en god og relevant behandling. 

Din journal bliver opbevaret i et arkivskab under lås eller i et sikret journalsystem, der ligger hos en databehandler, vi har en aftale med. Vi gemmer dine oplysninger i 5 år, i særlige tilfælde længere. Din journal bliver tilintetgjort 5 år efter du sidst har varet her, i særlige tilfælde senere. 

Du har ret til at få at vide, hvad der står i din journal. Kontakt din psykolog hvis du vil have en kopi. 

Vi har tavshedspligt, og det er kun os, der har adgang til din journal. 

Vi har pligt til at bryde vores tavshedspligt, hvis der er alvorlig risiko for, at du eller andre kommer til skade. 

Vi har pligt til at underrette kommunen, hvis vi er bekymrede for et barns trivsel og udvikling. 

Vi kan modtage og videregive oplysninger om dig fra din læge, dit forsikringsselskab og andre, hvis du har givet dit samtykke til, at de sender oplysningerne til os. 

Vi opbevarer oplysninger til brug for afregningsformål. De opbevares så længe det er nødvendigt, af hensyn til afregning og bogføring. 

Vi sender nødvendige regningsinformationer til betalere, hvis nogen (f.eks. sygesikringen, forsikring, arbejdsgiver, kommune) betaler dine samtaler helt eller delvist. 

Du har pligt til at informere os, hvis der skulle opstå ændringer i dine kontaktoplysninger. 

Hvis du er utilfreds med hvordan vi håndterer dine oplysninger, så sig det til din psykolog, og vi finder en løsning. Hvis du stadig er utilfreds, kan du klage til Datatilsynet (Datatilsynet, Carl Jacobsens Vej 35, 2500 Valby. www.datatilsynet.dk). 

SAMTYKKE TIL VIDEREGIVELSE AF OPLYSNINGER

Jeg giver hermed samtykke til at KonPsyk må videregive oplysninger til mit forsikringsselskab om antal af behandlinger og status på mit nuværende behandlingsforløb (i tilfælde af at det er en forældre/værge der skriver under, giver værgen tilladelse til behandling af personen der går i behandling). 

Samtykket om videregivelse af oplysninger kan til enhver tid tilbagekaldes. Du kan tilbagekalde dit samtykke skriftligt ved at sende en skriftlig erklæring herom til post@konpsyk.dk. 

Tilbagekaldelse af dit samtykke kan betyde at behandlingen hos KonPsyk ikke længere er mulig. 

Tilbagekaldelsen af samtykke berører ikke lovligheden af videregivelse af personoplysninger, der er baseret på dit samtykke inden tilbagetrækningen.

Ved for sent afbud, forbeholder vi os retten til at sende et afbudsgebyr.

Ved udeblivelse uden orientering herom, forbeholder vi os retten til at sende et afbudsgebyr.

Information on the Handling of Personal Data and Cancellations/No-shows

HANDLING OF PERSONAL DATA AT KONPSYK 

Welcome to KonPsyk. Here is some information we are required to provide to you under the EU Data Protection Law.

We are required to keep records. This means that we record and store information about you (e.g. name and CPR number) and what we discuss (e.g. your social circumstances and your health). The purpose is to provide you with appropriate and relevant treatment.

Your record is stored in a locked filing cabinet or in a secure record system hosted by a data processor with whom we have an agreement. We keep your information for 5 years, in special cases longer. Your record is destroyed 5 years after your last visit, in special cases later.

You have the right to know what is in your record. Contact your psychologist if you would like a copy.

We are bound by confidentiality, and only we have access to your record.

We are required to break confidentiality if there is a serious risk that you or others may come to harm.

We are required to notify the municipality if we are concerned about a child’s well-being and development.

We may receive and disclose information about you from your doctor, insurance company, and others if you have given your consent for them to send us the information.

We store information for billing purposes. This information is kept for as long as necessary for invoicing and accounting.

We send necessary billing information to payers if someone (e.g. health insurance, insurance, employer, municipality) pays for your sessions in full or in part.

You are required to inform us if there are any changes to your contact details.

If you are dissatisfied with how we handle your information, please tell your psychologist, and we will find a solution. If you are still dissatisfied, you may file a complaint with the Danish Data Protection Agency (Datatilsynet, Carl Jacobsens Vej 35, 2500 Valby. www.datatilsynet.dk).

CONSENT TO DISCLOSURE OF INFORMATION

I hereby consent to KonPsyk disclosing information to my insurance company about the number of treatments and the status of my current course of treatment (if this is signed by a parent/guardian, the guardian grants permission for treatment of the person receiving treatment).

Consent to the disclosure of information can be withdrawn at any time. You can withdraw your consent in writing by sending a written declaration to post@konpsyk.dk.

Withdrawal of consent may mean that treatment at KonPsyk is no longer possible.

The withdrawal of consent does not affect the legality of the disclosure of personal data that took place on the basis of your consent before its withdrawal.

CANCELLATIONS/NO-SHOWS

In case of cancellation of an appointment, regardless of the reason, please contact KonPsyk by email (post@konpsyk.dk) or phone (71948940) as soon as possible and no later than 12:00 p.m. the day before.
If cancellations are made too late, we reserve the right to charge a cancellation fee.

In case of no-show without prior notice, we reserve the right to charge a cancellation fee.
