Behandling af dine personoplysninger – samtykke

Jeg giver hermed mit samtykke til at Psykolog Anna Sørensen Rask må indsamle og behandle mine personoplysninger. Personoplysninger omfatter følgende typer:

• Navn
• E-mail adresse
• Kontaktinformation
• CPR-nummer
• Helbredsoplysninger

Oplysningerne opbevares GDPR-sikret elektronisk hos Terapeut Booking (https://terapeutbooking.dk/) i henhold til EUs persondatalov. Det betyder følgende: 

• Det er lovpligtigt som autoriseret psykolog at skrive journal. Derfor fører jeg også journal ved dit forløb. Formålet er at give dig en god behandling. 
• Din journal bliver opbevaret i 5 år i et GDPR-sikret online system og i særlige tilfælde længere. 
• Det er kun psykolog Anna Sørensen Rask, der har adgang til din journal. 
• Du har ret til at vide, hvad der står i din journal, hvis du ønsker det. 
• Jeg må ikke slette det skrevne i din journal, kun tilføje rettelser. 
• Hvis du har givet samtykke, kan jeg videregive informationer om dig til din læge eller andre behandlere - men kun hvis du har givet samtykke til det. 
• Jeg har pligt til at bryde min tavshedspligt, hvis jeg vurderer, at du er i alvorlig fare for dig selv eller andre. 
• Jeg har pligt til at underrette kommunen, hvis jeg er alvorlig bekymret for dit barns trivsel eller udvikling. 
• Jeg sender årligt oplysninger til min revisor ift. at udarbejde et årsregnskab. Min revisor har også tavshedspligt. 
• Hvis du er utilfreds med min måde at håndtere data på, så kontakt mig endeligt. Er du fortsat utilfreds, kan du klage til Datatilsynet. 

Jeg er klar over, at Anna Sørensen Rask har tavshedspligt. 

Jeg er bekendt med, at jeg skal melde afbud senest 24 timer før. Ellers afregnes fuld takst. Hvis samtalerne dækkes af en forsikringsaftale, kan der gælde andre regler, som oplyses ved første samtale. 

Samtykke til videregivelse af oplysninger (KUN hvis du er forsikringsklient):

Til brug for afregning og dokumentation i henhold til bogføringsloven, ansøgning om og tildeling af timer til behandling, forlængelse af bevillingsperiode giver jeg samtykke til at Anna Sørensen Rask må videregive oplysninger til forskringensinstansen om antal af behandlinger og status på mit behandlingsforløb - både løbende og ved afslutning af behandlingsforløb. Disse oplysninger videregives til forsikringsinstansen med henblik på rapportering til kunden til det nævnte formål. 

Samtykket kan til enhver tid tilbagekaldes. Konsekvensen af tilbagekaldelse af dit samtykke er, at du ikke kan få dækket behandlingen via forsikringen.

Tilbagekaldelsen af samtykke berører ikke lovligheden af videregivelse af personoplysninger, der er baseret på dit samtykke inden tilbagetrækningen.

Declaration of consent

Processing of your personal data – consent

I hereby give my consent for Psychologist Anna Sørensen Rask to collect and process my personal data. Personal data includes the following types:

Name
Email address
Contact information
CPR-number
Health information

The information is stored electronically at Therapist Booking (https://terapeutbooking.dk/) in compliance with the EU personal data protection law. It means the following: 

• It is a legal requirement for an authorised psychologist to keep clinical records. Therefore, I also keep a record for your treatment process. The purpose is to provide you with good and safe treatment.
• Your clinical record is stored for 5 years in a GDPR-secured online system and, in special cases, for a longer period.
• Only psychologist Anna Sørensen Rask has access to your record.
• You have the right to know what is written in your record if you wish.
• I am not allowed to delete what has been written in your record, only to add corrections.
• If you have given consent, I may share information about you with your doctor or other healthcare providers – but only if you have given explicit consent.
• I am required to break confidentiality if I assess that you are in serious danger of harming yourself or others.
• I am required to notify the municipality if I am seriously concerned about your child’s wellbeing or development.
• I send annual information to my accountant for the purpose of preparing my annual accounts. My accountant is also bound by confidentiality.
• If you are dissatisfied with how I handle your data, please feel free to contact me. If you remain dissatisfied, you can file a complaint with the Danish Data Protection Agency (Datatilsynet).

I am aware that Anna Sørensen Rask has a duty of confidentiality.

I am aware that I must cancel at least 24 hours before. Otherwise, the full rate will be charged. (If I have the sessions covered by your insurance company, the rules may differ, if so you will be informed during the 1st session).

Consent to disclosure of information (ONLY if you are an insurance client): 

For settlement and documentation in accordance with the Bookkeeping Act, application for and allocation of hours for treatment, extension of the grant period, I consent to Anna Sørensen Rask passing on information to your insurance company about the number of treatments and the status of my treatment course - both on an ongoing basis and at the end of the treatment course. This information is passed on to your insurance company for the purpose of reporting to the insurance company's customer for the stated purpose. Consent can be revoked at any time. The consequence of revoking your consent is that you cannot receive insurance-covered treatment at Psychologist Anna Sørensen Rask. The withdrawal of consent does not affect the legality of the disclosure of personal data based on your consent before the withdrawal.

Withdrawal of your consent
