Client Consent and Coaching Agreement
(for Resurface International)
1. Purpose of Coaching
Life coaching is a professional relationship that helps you identify goals, clarify values, and make desired changes in your personal or professional life. Coaching is not therapy, counseling, or medical treatment. It does not diagnose or treat mental health conditions. If such concerns arise, you may be referred to an appropriate licensed professional.
2. Nature of the Relationship
Coaching is a collaborative process focused on your goals. You, as the client, are fully responsible for your own choices, actions, and results. The coach’s role is to support, challenge, and guide you through reflection, feedback, and structured techniques.
No specific outcomes can be guaranteed.
3. Confidentiality
All information shared in sessions is kept strictly confidential and will not be disclosed to any third party without your written consent, except in the following situations:
If you disclose intent to harm yourself or others.
If there is suspected abuse or neglect of a child, elder, or vulnerable adult.
If required by law or court order.
Session notes and communication are securely stored using encrypted digital systems (e.g., EasyPractice or other GDPR-compliant platforms).
4. Fees and Payment
Fees are communicated in advance and payable prior to each session or according to the agreed package. Cancellations made less than 24 hours before a scheduled session may be charged in full unless due to an emergency.
5. Duration and Termination
You may discontinue coaching at any time. The coach may also end the coaching relationship if it appears that coaching is no longer appropriate or effective, or if ethical or boundary concerns arise.
6. Risks and Benefits
Coaching may involve exploring personal and emotional topics that can feel uncomfortable. While this can be part of positive change, please communicate openly about any concerns during the process. Benefits may include increased clarity, confidence, motivation, and better decision-making.
7. Communication and Boundaries
Coaching sessions typically take place via secure video, phone, or in-person (where available). Email or text communication outside of sessions should be limited to scheduling or brief check-ins.
8. Data Protection
Resurface International complies with international privacy standards, including the GDPR. Personal data (name, email, contact details, and notes) are used only for coaching purposes and securely stored. You have the right to access, correct, or request deletion of your data at any time.
