Når du godtar denne avtalen, inngår du et samarbeid med meg om gestaltterapi på følgende betingelser:
Rett til å klage: Som medlem av NGF er jeg forpliktet til å utøve min virksomhet i henhold til gjeldende Etiske prinsipper for gestaltterapeuter MNGF, og til de lover, forskrifter og regler som til enhver tid gjelder. Dersom du/ klienten mener at jeg/ gestaltterapeuten har opptrådt på en uetisk måte, kan du klage til Faglig Etisk Råd i Norsk gestaltterapeutforening (NGF).
Taushetsplikt: Jeg opptrer lojalt med mine klienter og overholder taushetsplikten. Taushetsplikten gjelder også i forbindelse med veiledning og etter at terapien er avsluttet. Bare du selv, eller den med foreldreansvar for mindreårige (under 16 år), kan gi samtykke til at opplysninger underlagt taushetsplikt kan gis til en tredjepart.
Varsling og anmeldelsesplikten i straffelovens § 139 er gjeldende, og taushetsplikten kan brytes hvis en persons liv eller helse er i akutt fare, eller en person er til fare for andres liv eller helse.
Dokumentasjon: Jeg fører journal etter våre møter. Du har rett til innsikt i all data jeg lagrer om deg. Journalnotater er lagret i Easypractice, og ingen andre har tilgang til disse. Se øvrig informasjon om håndtering av av persondata nedenfor.
Betaling: Pris ved avtaleinngåelse er
kr 1080,- (864,- + 25% moms) pr time individuell terapi av 60 minutter
kr 1500,- (1200,- + 25% moms) pr time individuell terapi av 90 minutter
kr 1750,- (1400,- + 25% moms) pr time parterapi av 75 minutter
Prisen kan reguleres med 3 mnd varsel.
Timer avbestilt senere enn 24 timer før avtalt tid, faktureres som full time.
Betaling utføres gjennom Vipps eller ved faktura tilsendt på email etter timen.
Dersom en tredjepart er ansvarlig for betaling av timer, ber jeg om at vedkommende sender en bekreftelse pr email til marianne@spireterapi.no.
Kommunikasjon: Ved inngåelse av avtale med meg, samtykker du til kontakt gjennom email og sms for avtale om timer, fakturering og andre praktiske henvendelser. Dersom annet er ønskelig, vennligst gi beskjed pr email.

Databehandlerne vil kun bruke personopplysningene til det formålet de ble samlet inn for og for å utføre tjenestene for virksomheten. Utlevering av personopplysninger til myndigheter forkommer kun i henhold til lov og gjeldende regelverk, der eventuelt taushetsplikten opphører. All annen utlevering skjer kun etter skriftlig instruks og samtykke fra den registrerte.
Innsyn i personopplysninger
Enkeltpersoner kan til enhver tid få innsyn i sine personopplysninger, kopi av journal og andre opplysninger. Virksomheten oppbevarer personopplysninger så lenge det er nødvendig for å oppfylle behandlingsformålet, og i henhold til regnskapsloven.
Tilbaketrekking av samtykke til behandling av dine personopplysninger

Dersom du mener at virksomhetens behandling av personopplysninger er i strid med relevante personvernbestemmelser har du rett til å sende inn klage til virksomheten direkte, Datatilsynet eller annen relevant tilsynsmyndighet.
I am a member of the Norwegian Gestalt Therapy Association (NGF). This means that I am committed to quality assurance measures such as receiving regular supervision (confidentiality is maintained) and carrying liability insurance. By accepting this agreement, you enter into a collaboration with me for gestalt therapy under the following conditions:
Right to complain: As a member of NGF, I am obliged to practice in accordance with the current Ethical Principles for Gestalt Therapists MNGF, as well as all applicable laws, regulations, and rules. If you, as the client, believe that I have acted unethically, you may file a complaint with the Professional Ethical Council of the Norwegian Gestalt Therapy Association (NGF).
Confidentiality: I work loyally with my clients and adhere to strict confidentiality. This obligation also applies in connection with supervision and continues after therapy has ended. Only you, or the person with parental responsibility for minors (under 16 years), can consent to information covered by confidentiality being shared with a third party.
Exceptions: In accordance with Section 139 of the Norwegian Penal Code, confidentiality may be broken if a person’s life or health is in immediate danger, or if someone poses a risk to the life or health of others.
Documentation: I keep records of our sessions. You have the right to access all data I store about you. Session notes are stored in EasyPractice, and no one else has access to them. See further information on handling of personal data below.
Payment: The fee at the time of agreement is:
NOK 1080 (864 + 25% VAT) per 60-minute individual session
NOK 1500 (1200 + 25% VAT) per 90-minute individual session
NOK 1750 (1400 + 25% VAT) per 75-minute couples session
Fees may be adjusted with 3 months’ notice.
Sessions cancelled less than 24 hours before the scheduled time will be charged in full.
Payment is made via Vipps or by invoice sent by email after the session.
If a third party is responsible for payment, I ask that they send written confirmation by email to marianne@spireterapi.no.
Communication: By accepting this agreement, you consent to being contacted via email and SMS regarding appointments, invoicing, and other practical matters. If you prefer other channels, please notify me.

These processors will only use personal data for the purposes it was collected and to provide services for the practice. Disclosure of personal data to authorities occurs only in accordance with law and current regulations, where confidentiality no longer applies. Any other disclosure is only carried out upon written instruction and consent from the client.
Access to personal data You may at any time request access to your personal data, copies of your records, and other related information. The practice stores personal data for as long as necessary to fulfill the treatment purpose and in accordance with accounting regulations.
Withdrawal of consent You may withdraw your consent at any time by contacting Spire Therapy | Gestalt Therapist Marianne Winnæss at marianne@spireterapi.no or by phone: +47 47022825.
If you believe that the processing of your personal data is in violation of applicable data protection laws, you have the right to file a complaint directly with the practice, with the Norwegian Data Protection Authority (Datatilsynet), or another relevant supervisory authority.
