Behandling af dine personoplysninger – samtykke

Jeg giver hermed mit samtykke til at Psykolog Tina Bjarnskov må føre elektronisk journal. Dette indebærer at Tina Bjarnskov må indsamle og gemme mine personoplysninger. Personoplysninger omfatter følgende typer:

• Navn
• Kontaktinformation
• CPR nummer
• Helbredsoplysninger (herunder mentalt helbred)

Oplysningerne opbevares elektronisk hos Terapeut Booking (www.terapeutbooking.dk).

Tilbagetrækning af dit samtykke til behandling af dine personoplysninger

Du kan altid trække dit samtykke tilbage. Dette kan du gøre ved at kontakte Psykolog Tina Bjarnskov på e-mail: kontakt@tinabjarnskov.dk

Afbudsregler

Afbud skal ske 24 timer før sessionen og 72 timer før sessioner på mandage, ellers opkræves fuld pris for sessionen. 

Processing of your personal data - consent

I hereby give my consent for Psychologist Tina Bjarnskov to maintain an electronic client record. This includes permission for Tina Bjarnskov to collect and store my personal data. Personal data includes the following types:

• Name
• Contact information
• Social Security number
• Health information including mental health.

The information is stored electronically at Terapeut Booking (www.terapeutbooking.dk). 

Withdrawal of your consent to the processing of your personal data

You can always withdraw your consent. You can do this by contacting Psychologist Tina Bjarnskov by e-mail: kontakt@tinabjarnskov.dk

Cancellation policy

Cancellations made less than 24 hours before the session or less than 72 hours before sessions on Mondays will be charged full fee for the session.
